Job Description: Reporting to a Manager in Primary Health Care, in the North Network, the Chronic Disease Management Diabetes Nurse Educator responsibilities include; coordinating education for clients living with diabetes, providing best practice in diabetes management as part of a multidisciplinary team, providing individual inpatient and outpatient education for clients, facilitating group classes in the outpatient setting, promoting self-management education and participating in departmental activities such as program development, planning, quality improvement and research.

Experience
- 5 years recent nursing experince
- 3 years clinical experience with patients/families with diabetes disease mgmt
- 1 yr experience in adult education/group facilitation
- Experience working with complex patients and families
Education
- Bachelor of Science in Nursing (BScN)
Licenses
- Valid Class 5 driver's license
Other Education and Training
- Current Certification as a Diabetes Educator (CDE) is required
- Basic Life Support Certificate (BLS)

- Professional membership with Diabetes Canada is desired
- Must possess a valid driver's license and have a vehicle available for work.
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

- 2 years of employment/Membership with the Diabetes Education Section is an asset
