Make an impact at the front door of healthcare. Bring your clinical judgment beyond the bedside and help patients navigate pivotal transitions in care while supporting safety, access, and efficient patient flow.

As an Emergency Department RN Case Manager at UVA Health, you will combine your acute care expertise with real-time assessment, discharge planning, and care coordination. Working alongside emergency physicians, nurses, social workers, rehabilitation providers, and community partners, you will help patients receive the right care, at the right time, in the right setting.

The Role

This is a highly collaborative nursing role for an experienced clinician who excels at pattern recognition, critical thinking, and navigating complex care needs. You will assess patients early in their Emergency Department journey, identify clinical and social barriers, and develop safe, practical plans that support continuity of care across settings.

Your average day will include:

  • Conducting timely patient assessments and identifying care coordination needs early in the care journey
  • Applying independent clinical judgment to complex medical, surgical, trauma, critical care, and specialty populations
  • Coordinating safe dispositions with physicians, nurses, social workers, inpatient teams, rehabilitation providers, post-acute facilities, home health agencies, and community resources
  • Removing barriers to discharge while advocating for patients and families during critical moments
  • Participating in interdisciplinary collaboration and real-time care planning
  • Using Epic and evidence-based case management practices to support continuity, quality, and safety
  • Helping optimize patient flow without losing sight of each patient’s individual needs and circumstances

The Emergency Department offers variety, complexity, and continuous learning. No two days are the same, and your expertise will directly influence discharge planning, healthcare access, patient experience, and system-wide care delivery.

This opportunity also offers a primarily weekday schedule with more predictable hours than many traditional bedside roles. A weekend rotation is included, with scheduled time off surrounding worked weekends.

Why UVA Health

UVA Health offers the clinical depth of an integrated academic health system and the collaborative culture of a Magnet-recognized nursing organization. Within the Emergency Department, you will join an interdisciplinary team shaping the future of emergency care in a modern 70-bed environment designed to improve patient and clinical workflows.

RN Case Managers are valued as essential partners in clinical decision-making, patient flow management, and discharge planning. You will have the opportunity to practice at the top of your license while developing a broad perspective across the entire continuum of care.

  • Supportive leadership committed to professional growth and career development
  • Comprehensive onboarding and training for nurses transitioning into case management
  • Support for ACM-RN certification, continuing education, and conference attendance
  • Mentorship and preceptor opportunities
  • Pathways to grow as a subject matter expert or advance into leadership
  • Exposure to diverse patient populations and multiple care settings
  • Collaboration with practitioners, faculty, students, and healthcare professionals in an academic environment grounded in innovation and evidence-based practice

Experienced ED, ICU, Med/Surg, trauma, and acute care nurses will find an opportunity to translate bedside expertise into broader clinical impact while building a long-term career in case management.

Relocation assistance may be available for qualified candidates.
