Bring your clinical judgment, communication skills, and commitment to continuity of care to a role where expert nursing coordination directly supports patients managing complex endocrine and metabolic conditions.

As an RN Care Coordinator with UVA Health’s Fontaine Endocrine Clinic, you will help patients navigate ongoing care for diabetes, thyroid, adrenal, pituitary, bone health, gender health, and general endocrine disorders. This full-time, salaried opportunity combines ambulatory nursing, interdisciplinary collaboration, and hybrid flexibility within a cohesive and supportive team.

The typical schedule is 8:00 a.m. to 4:30 p.m., with no on-call requirement. Occasional coverage may require hours of 7:45 a.m. to 4:15 p.m. or 9:00 a.m. to 5:30 p.m. The role typically includes one remote workday per week, with the potential for additional remote flexibility when staffing permits.

The Role

You will serve as a key clinical connection among patients, providers, nurses, and the clinic’s access team. Using strong assessment skills and sound clinical judgment, you will manage patient needs remotely and in the ambulatory setting while helping ensure timely, coordinated care.

Your responsibilities will include:

  • Providing nursing telephone triage for patients with endocrine and metabolic conditions
  • Managing Epic In Basket messages, e-faxes, durable medical equipment requests, and other care-coordination workflows
  • Reviewing and downloading glucose readings as needed to support clinical decision-making
  • Communicating with patients and providers regarding symptoms, treatment needs, prescriptions, equipment, and follow-up care
  • Coordinating care for an assigned patient population across multiple endocrine specialties
  • Providing cross-coverage for other RN Care Coordinators and the infusion nurse
  • Supporting charge nurse or late nurse coverage when patient-care or staffing needs require it
  • Contributing to new ambulatory workflow implementation and ongoing evaluation of clinical phone volumes and nursing workloads
  • Applying flexibility, initiative, and collaborative problem-solving as workflows and patient needs evolve

You will be supported as you develop independence in the care-coordination role. Success means becoming confident in managing your responsibilities while building the versatility to provide cross-coverage, with experienced colleagues close by when guidance is needed.

Why UVA Health

At UVA Health, you can advance your nursing practice in an academic health system where interdisciplinary expertise, evolving clinical knowledge, and patient-centered care come together. The Fontaine Endocrine Clinic supports a broad and clinically diverse patient population, giving you the opportunity to deepen your expertise while making a meaningful impact on patients managing chronic and complex conditions.

Here, you will benefit from:

  • A collaborative, inclusive team culture grounded in open communication, shared discussion, and collective problem-solving
  • Supportive providers, experienced nursing colleagues, and a dedicated access team
  • Exposure to a wide range of endocrine and metabolic conditions, including diabetes, thyroid, adrenal, pituitary, and bone disorders
  • Opportunities to strengthen expertise in ambulatory triage, care coordination, glucose data, and durable medical equipment workflows
  • A team actively improving ambulatory workflows and evaluating clinical demand to better support patients and staff
  • No on-call requirement and meaningful hybrid flexibility
  • A role where nursing judgment and communication directly influence access, continuity, and the patient experience
  • An environment well suited to nurses who value teamwork, camaraderie, adaptability, and continuous learning

Candidates with adult medical-surgical nursing experience are encouraged to apply. Previous ambulatory, endocrinology, or care-coordination experience is valued, but the team is also open to nurses whose transferable clinical skills prepare them to succeed in this setting.

Relocation assistance may be available for qualified candidates.
