**LICENSED PRACTICAL NURSE (LPN) - PALLIATIVE CARE COORDINATOR - HOSPICE SERVICES**

**KEY RESPONSIBILITIES:**

· Responsible for taking and processing Palliative Care referrals.

· Provide support to the Palliative Care Nurse Practitioner and Medical Director.

· Will schedule visits to support goals of care as ordered by MD.

· Coordinate and communicate patient care goals between all providers.

· Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.

· Ensure communication of assessment and reassessment findings to attending physician.

· Complete all documentation accurately, timely, and in accordance with company policy and procedure.

· Inform palliative team of all new admissions and discharges.

· Coordinate referrals utilizing the referral process.

· Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

· Run billing report and address validation holds/ billing errors

**LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:**

· Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice

· A minimum of three (3) years of nursing experience

· Palliative care, hospice, or home health care experience is preferred

· CHPN (Certified Hospice and Palliative Nurse) is preferred
