The job holder is responsible for conducting care assessments and planning and managing residents’ care in accordance with established protocols and evidence-based practices, under appropriate supervision.
About the Role
The job holder is expected to demonstrate competency in coordinating care across the healthcare continuum and providing holistic care through effective collaboration with interdisciplinary teams and relevant community partners. The job holder facilitates care transitions and coordination based on residents’ individual needs and preferences to encourage self-management and continuity of care.
The Staff Nurse also contributes to evidence-based practice, innovation, quality improvement and/or research projects.
Job Description
As a Senior / Staff Nurse in Community Care, you will be required to provide nursing care and health coaching in the following areas:
- Health promotion and illness prevention through patient education
- Primary care, including conducting basic assessments and providing health education to the general public
- Home care services for patients transitioning from hospital to home
- Any other community-related duties as assigned
Job Requirements
- Diploma or Degree in Nursing and valid registration with the Singapore Nursing Board
- Minimum of two to three years of relevant work experience in a community setting
- Ability to work independently and collaboratively as part of a team
- Strong organisational, coordination, interpersonal and communication skills
- Ability to converse in local languages is preferred
