About the Role
We have exciting opportunities available for registered nurses to join Monash Children’s at Home based in Clayton at the Monash Children’s Hospital. This is a Part-time, ongoing opportunity.
Reporting to the Nurse Unit Manager, you will work largely independently, providing a safe, effective alternative model of care for Newborn/Paediatric patients needing ongoing nursing management at home. - Planning, evaluating and maintaining a high standard of safe and effective care for children
- Conducting comprehensive health assessments and educating those involved in care planning, direct care and discharge planning in the ambulatory setting
- Ensuring timely discharge planning to facilitate patients’ safe and effective transition from hospital to home
- Demonstrating your commitment to developing, promoting and progressing continuous quality improvement initiatives
- Role modelling excellence in safe nursing practice consistent with our expectations of leaders across Monash Health
