Advanced Heart Failure & Cardiac Transplant APP
Join a collaborative inpatient cardiovascular team caring for patients with advanced heart failure, cardiogenic shock, cardiac amyloidosis, pulmonary hypertension, and those undergoing cardiac transplantation. This role provides comprehensive inpatient management of complex cardiovascular patients, including care in the intensive care unit (ICU) and specialized heart failure and transplant services.

The APP will work alongside a multidisciplinary team of physicians and advanced practice providers to deliver high-quality, patient-centered care. Responsibilities include the evaluation and management of patients with advanced heart failure, cardiogenic shock, cardiac amyloidosis, pulmonary hypertension, and pre- and post-cardiac transplant patients in both acute care and ICU settings.

This position consists of 12-hour shifts (7:00 AM-7:00 PM), with an average of one to two weekend rotations per month and one to two holidays per year. The APP will be an integral member of a highly specialized team dedicated to caring for some of the most complex cardiovascular patients in an academic medical environment.

Key Responsibilities:

  • Manage patients with advanced heart failure, cardiogenic shock, cardiac amyloidosis, and pulmonary hypertension in inpatient and ICU settings
  • Provide care for patients before and after cardiac transplantation
  • Collaborate with cardiologists, cardiac surgeons, nursing staff, and multidisciplinary care teams
  • Participate in care coordination, treatment planning, and patient/family education
  • Deliver evidence-based, high-quality cardiovascular care in a fast-paced academic environment
  • Support continuity of care across hospitalization and transition planning processes

During the selection process you may participate in an OnDemand (pre-recorded) interview that you can complete at your convenience. During the OnDemand interview, a question will appear on your screen, and you will have time to consider each question before responding. You will have the opportunity to re-record your answer to each question – Mayo Clinic will only see the final recording. The complete interview will be reviewed by a Mayo Clinic staff member and you will be notified of next steps.
