**Location Detail:** 15 Armstrong Road Shelton (10505)

**Shift Detail:** Monday–Friday: Shift availability between 8:00 AM–10:00 PM, with flexibility in assigned shift times. Every other weekend: 8-hour shift; Rotating weekend coverage required every other weekend

**Job Summary**

Under the supervision of a Physician, Registered Nurse, or Clinician, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice.

_This role requires coverage at regional Urgent Care locations bringing variety to your workday and the chance to collaborate across multiple teams and communities. Reliable transportation is needed._

- Collaborates with members of health care team to assist patients in achieving established outcomes of care
- Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
- Performs therapeutic measures, under the direction of an RN or Provider, to assist in achieving established patient care outcomes.

#### Qualifications

- Current Connecticut Licensed Practical Nurse (LPN) license required
- CPR certification
- Medical/Surgical office experience preferred
- Specialization and/or specific work experience may be required
