- Oversee the daily operations of the case management department and ensure efficient and effective delivery of services.
- Develop and implement policies and procedures for case management services in accordance with organizational standards and regulations.
- Hire, train, and supervise a team of case managers, providing guidance and support to ensure high-quality performance.
- Collaborate with other departments to develop and implement strategies for improving patient outcomes and achieving organizational goals.
- Monitor and analyze data related to case management services, identify areas for improvement, and implement solutions to enhance efficiency and effectiveness.
- Ensure compliance with all applicable laws, regulations, and standards related to case management services.
- Serve as a liaison between the case management department and other healthcare providers, community organizations, and insurance companies.
- Develop and maintain relationships with external stakeholders to enhance the quality of care and resources available to patients.
- Lead and participate in interdisciplinary meetings to discuss patient cases and develop care plans.
Education Qualifications
- Bachelor’s Degree in nursing Required or
- Master’s degree in social work Required
Experience Qualifications
- Minimum of five years' relevant experience of hospital experience recommended. Required
- Minimum of three years' relevant experience of case management supervisory experience Preferred
Licenses and Certifications
- Current RN Required or
- Social work licensure in applicable state. Required
- Certification in Case Management Required or
- plan in place to obtain within 2 years of hire date
Hours: 40per weeks
Schedule: Monday-Friday
