Provide partner services to new and existing HIV infected patients. - Work collaboratively with the medical care team providing patient counseling for management of medical treatment care plan. - Employ social networking strategies with new and existing HIV-infected patients to bring additional social contacts in for HIV testing. - Provide prevention counseling/strategies such as CLEAR, ARTAS, or Partners in Health for patients and their families. - This position’s role is key in the implementation of multi-disciplinary care. **Experience Required** Minimum of 1 year experience in providing HIV prevention or care services to people living with HIV. Experience counseling HIV (+) clients on connecting to available services.
