The LPN LTC is responsible for providing specialized care to patients in a long-term care setting. Working under general supervision, this role assesses, plans, and implements interventions to promote optimal healing and prevent complications. - Conducts comprehensive assessments of patients' needs to develop individualized care plans.
- Performs treatments following established protocols practices.
- Monitors and evaluates the progress of patients, documenting findings accurately and communicating any changes or concerns to the healthcare team.
- Educates patients, families, and caregivers on proper care techniques, prevention strategies, and signs of complications.
- Collaborates with physicians, registered nurses, and other healthcare professionals to coordinate care, ensure continuity, and optimize patient outcomes.
- Maintains accurate and up-to-date documentation of assessments, interventions, and outcomes in the electronic medical record system.
- Performs other duties as assigned.
Education
- Required: High School Diploma or equivalent; Diploma in Practical Nursing.
Experience
- Preferred: 1 year acute care experience
Licensure/Certification/Listing
- Required: Current LPN license in North Carolina; BLS (CPR)-American Red Cross or AHA Healthcare Provider correct
