Job Description:
Schedule: Full Time
Shift: Days 7am-7pm with every other Weekend
Location: Covering our Stuart F Meyer Inpatient Unit, Palm Coast

- Follows CDC isolation guidelines, demonstrates appropriate use of PPE, performs hand hygiene according to policy, and enforces safety regulations Implements care per the Plan of Care.
- Provides direct patient care to ensure comfort and meet changing needs.
- Educates patients and families on end-of-life care and issues.
- Prepares patients and families for approaching death.
- Promotes patient comfort through appropriate interventions.
- Provides emotional support to patients and families.
- Administers medications and treatments as prescribed.
- ssesses and monitors patient conditions regularly.
- Communicates effectively with patients, families, and healthcare team members.
- Documents patient care activities in compliance with Medicare regulations.
- Participates in interdisciplinary team meetings and care planning.
- Maintains accurate and timely patient records.
- Other duties as assigned.
Knowledge, Skills, and Abilities:

Field of Study:

Work Experience:
- 1+ years experience in the health care field [Required]

Licenses and Certifications:
- Licensed Practical Nurse (LPN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Driver's License (DL) [Required]
- Auto Insurance (AL) [Required]
- Certified Hospice and Palliative Nurse (CHPN) [Preferred]
